Ellansé vs filler: why we don’t think one is simply “better”

What you need to know

  • Ellansé and hyaluronic acid filler are not simply two versions of the same treatment.
  • HA filler mainly adds or reshapes volume; Ellansé also stimulates new collagen formation.
  • HA filler has an important advantage: it can usually be dissolved if needed. Ellansé cannot be routinely reversed in the same way.
  • Ellansé may make more sense when longer-term structural support is the goal, but it is not automatically the better choice.
  • The right treatment depends more on your anatomy, treatment area and long-term plan than on which product lasts longer.

If you are comparing Ellansé vs filler, the first thing to know is that Ellansé is technically a type of injectable filler too. The useful comparison is really Ellansé versus conventional hyaluronic acid, or HA, filler. They can both restore or reshape facial volume, but they behave differently after injection. This article explains those differences, including longevity, reversibility, collagen stimulation and the situations where we may choose one over the other.

Ellansé vs filler is not really a question of which one is stronger

One of the most common questions we hear is:

“Doctor, Ellansé or filler — which one is better?”

We understand why people ask. If two treatments can both be used for facial contouring, it seems reasonable to compare them like two competing products.

But that is not how we approach it.

A patient who would benefit from HA filler does not automatically become a better candidate for Ellansé simply because Ellansé may last longer.

And someone who is suitable for Ellansé may not get the same type of result by simply injecting more HA filler.

The more useful question is:

What are we trying to achieve, and what material makes the most sense for that particular face?

That distinction matters more than the name on the syringe.

The biggest difference happens after the injection

Conventional HA fillers are gels made from hyaluronic acid. Different products have different levels of firmness, flexibility and cohesivity, which is why doctors can select different HA fillers for different purposes.

Some are designed for projection and structural support. Others are softer and move more naturally with facial expressions.

Their primary job, however, is still to provide volume and shape.

Ellansé works differently.

It contains polycaprolactone, or PCL, microspheres suspended in a carboxymethylcellulose gel. The gel provides an initial volumising effect, while the PCL microspheres subsequently stimulate collagen formation in the surrounding tissue.

In simple terms:

HA filler mainly gives you the volume that was injected.

Ellansé provides initial correction while also encouraging your tissues to produce new collagen over time.

That is why Ellansé is often described as a collagen stimulator as well as a filler.

But collagen stimulation does not automatically mean a more beautiful result. Placement, facial proportions and patient selection still matter far more than the material itself.

Why we sometimes choose HA filler even when Ellansé lasts longer

Longevity sounds attractive.

Nobody wants to repeat a treatment more often than necessary.

But in aesthetic medicine, being longer-lasting is not always an advantage.

Sometimes we deliberately want flexibility.

A younger patient may still be experiencing changes in facial shape. Someone having their first injectable treatment may prefer something that can be adjusted more easily. Certain treatment goals may also require a softer or more adaptable material.

HA fillers are extremely versatile because their physical properties can vary substantially between products.

There is another important difference: HA filler can generally be broken down using hyaluronidase when medically appropriate. This reversibility is considered one of HA filler’s major advantages.

Ellansé does not have an equivalent enzyme that allows us to simply dissolve the PCL product on demand.

That changes the decision.

When we use Ellansé, we need to be particularly confident about the treatment plan, injection plane, amount and facial proportions before treatment.

At LS Clinic, having worked with a very high volume of Ellansé cases over the years has not made us want to use it on everyone.

If anything, experience teaches the opposite.

Sometimes the better decision is to use HA filler. Sometimes it is to use less product. And sometimes we tell a patient that we would rather not inject anything yet.

When Ellansé starts to make more sense

We tend to think about Ellansé when the objective goes beyond simply filling a small depression.

For example, a patient may have gradually lost facial support with age and wants to improve overall contour rather than make one feature noticeably larger.

In those situations, the combination of structural correction and collagen stimulation can be useful.

The initial CMC carrier gradually reduces after injection, while collagen produced around the PCL microspheres contributes to the longer-term effect.

This is also why judging Ellansé immediately after treatment can be misleading.

The face you see on day one is not necessarily the final way the tissues will behave months later.

Our preference with Ellansé is generally not to chase maximum volume.

We would rather ask where support has actually been lost, how restoring that support changes the face, and whether another syringe genuinely adds something useful.

This is also the thinking behind LS Clinic’s approach to facial contouring with Ellansé rather than treating every hollow or line independently. Ellansé 少女针

When ordinary HA filler may actually be the smarter choice

There are several situations where we may lean towards HA filler instead.

The first is when adjustability matters.

If a patient is unsure how much change they want, starting conservatively with HA can offer more flexibility.

The second is when we want very specific properties from the injectable material. HA fillers come in many formulations, ranging from relatively soft and flexible gels to products designed for stronger structural support.

The third is when reversibility is an important part of the treatment decision.

That does not mean HA filler is risk-free. All injectable fillers can cause side effects such as swelling, bruising, asymmetry, lumps, infection and, more rarely, serious vascular complications. Having a dissolving agent available does not replace good anatomy knowledge or safe injection technique.

It simply gives HA filler a management option that PCL-based Ellansé does not have in the same way.

For this reason, a consultation should never start with “Which filler do you want?”

It should start with an assessment of the face.

Does Ellansé last longer than HA filler?

Usually, Ellansé is chosen partly because of its longer-term collagen-stimulating effect.

HA filler duration varies considerably. Published literature commonly reports effects lasting from several months to around 18 months depending on the product, treatment area, injection technique and individual patient.

Ellansé has been developed in different formulations with different intended durations, and its effect is influenced by both the material and the collagen response it stimulates.

But we would be careful about choosing an injectable purely by asking which lasts the longest.

A treatment lasting two years in the wrong position is not better than a treatment lasting one year in the right position.

Longevity should be one part of the decision, not the decision itself.

What about Ellansé vs filler price in Malaysia?

This is another difficult comparison because the number of syringes is not necessarily equivalent.

One syringe of Ellansé and one syringe of HA filler may be used for completely different treatment plans.

Prices for both treatments in Malaysia can vary depending on the product used, amount required, doctor’s experience and complexity of the treatment.

Rather than comparing the cost per syringe alone, we suggest looking at the complete plan:

What is being corrected?

How much product is actually necessary?

How often would maintenance usually be considered?

Would a different treatment address the concern more directly?

For example, if the main concern is skin laxity rather than lack of volume, adding more injectable product may not be the best solution at all. A device-based tightening treatment such as Hifu may sometimes be discussed instead.

Good aesthetic treatment is not about finding somewhere to put another syringe.

Our view after years of working with Ellansé

LS Clinic has been recognised as a leading Ellansé user in Malaysia over several consecutive years.

But our conclusion from that experience is not that everyone should choose Ellansé.

It is that patient selection matters more as the treatment becomes longer-lasting.

Ellansé can be a very useful option when we want structural support together with gradual collagen stimulation.

HA filler can be a very useful option when precision, softness, versatility or reversibility is important.

And there are patients for whom we would recommend neither.

If the face does not need additional volume, adding volume simply because a treatment is popular can make the result look heavier rather than younger.

So when someone asks us “Ellansé vs filler — which one should I do?”, our answer usually begins with another question:

What exactly are we trying to improve?

Once that is clear, choosing the product becomes much easier.

A proper facial assessment with a qualified medical practitioner can help determine whether HA filler, Ellansé, another treatment or no injectable treatment at all is the more appropriate option for your anatomy and goals.

FAQ

Is Ellansé better than filler?

Not necessarily. Ellansé and conventional hyaluronic acid filler have different properties. Ellansé combines initial volume with PCL-driven collagen stimulation, while HA filler offers a wide range of textures and the additional advantage of reversibility. The better option depends on the treatment area, anatomy and desired result.

What is the main difference between Ellansé and HA filler?

HA filler primarily restores or reshapes volume using a hyaluronic acid gel. Ellansé contains PCL microspheres in a gel carrier and also stimulates new collagen formation over time, so its effect is not based solely on the material initially injected.

Can Ellansé be dissolved like hyaluronic acid filler?

Ellansé cannot be routinely dissolved with hyaluronidase in the way that HA filler can. This is one reason careful patient selection, treatment planning and injection technique are particularly important when using Ellansé.

How long does Ellansé last compared with filler?

HA filler commonly lasts several months to around 18 months depending on the product, area and individual. Ellansé is designed to provide a longer-term effect through PCL and collagen stimulation, although actual duration varies and should be discussed according to the specific product and treatment plan.

Is Ellansé more expensive than filler in Malaysia?

Treatment costs vary between clinics and depend on the product, number of syringes, treatment area and complexity of the procedure. Comparing price per syringe alone can be misleading because Ellansé and HA filler may be used in different ways and with different maintenance schedules.

Who should not choose Ellansé?

Ellansé may not be appropriate when a patient wants a highly adjustable or easily reversible treatment, when additional facial volume is not actually needed, or when the medical assessment identifies another treatment as more suitable. Suitability should be determined after an in-person consultation with a qualified medical practitioner.

The information here is meant to help you understand a treatment before you decide — it isn't a diagnosis or medical advice, and it can't replace seeing a doctor in person. Every face and every case is different, so results vary from person to person, and any treatment only goes ahead after an LS Clinic doctor has assessed that it's right for you.
